Juniper’s Connect4Life is a pioneering model of integrated care for seniors housing. As part of this model Juniper integrates care and service provided on-site by preferred ancillary providers through a single seamless program designed to enhance wellbeing, improve quality of life and contain costs. One important aspect of this model is providing medical care remotely through telemedicine. Modern technology enables doctors to consult patients by using video-conferencing tools, connecting patient and doctor in real time. Dr. Kristen Nebel, Geriatric Specialist with UPMC Pinnacle, held a virtual office visit recently with one of her patients who resides at Juniper Village at Lebanon. Tia Stokes, RN and Director of Wellness at the community, played a vital role in connecting Dr. Nebel and her patient through this technology at our onsite Redwood clinic. Vital signs, questions and concerns were able to be communicated and checked on site with Tia’s help. Juniper is excited to be able to provide this innovative service to our residents now and in the future.
