Juniper Communities Earns National Award for Providing Integrated Care On-Site for Senior Living Residents

BLOOMFIELD, NJ (November 4, 2021) – Juniper Communities has been selected by Mather Institute as an honorable mention of the Promising Practices Award.

Organizations from around the continent working with older adults in a variety of settings sent in submissions that are moving away from conventional practices by developing and implementing new and innovative approaches. Even in this most unusual time, powerful ideas and practices continue to move the senior living industry forward. Juniper Communities was chosen based on their unique model that offers residents in their senior living communities integrated medical care on-site.

“The Promising Practices Award honors Juniper Communities for its solution to the challenges of escalating demand for health care, rising health care costs, and poor outcomes,” said Cate O’Brien, PhD, AVP and Director, Mather Institute. “The awards honor true leaders in ideation and implementation. We hope these award-winning projects will serve as a catalyst for organizations across the country and around the world to spark innovation for their own programs.”

Juniper Communities’ Connect4Life program integrates care and service delivered by community staff with that offered by outside clinical providers. The result is an integrated team, better outcomes, cost savings, and a seamless on-site health care experience for residents.

“When we pioneered Connect4Life it was to create a program that lowered hospital readmissions for our frail seniors and significantly lower costly inpatient services. We also sought to create a model that could be replicated across our communities as well as the senior living industry as a whole.
